Continuum of Care

Mission
Support Native American families through respectful, quality care built on pride, communication, and trust for a healthier community.
Purpose
Services are designed to support individuals across the continuum of care, from preventive and supportive services to complex long-term care and home health needs. The Oneida Comprehensive Health Continuum of Care Department serves as the primary hub for coordinating in-home, community-based, and long-term care services for Oneida community members, helping individuals maintain their independence, health, and quality of life. Staff collaborate with providers, nurses, pharmacists, caregivers, and other support services to coordinate care for individuals with medical, behavioral, social, and long-term care needs. By integrating multiple programs and services, the department ensures individuals receive the right level of care at the right time while remaining in their homes and communities whenever possible.
